Intra Arterial Heparin Flushing Increases Cereberal Blood Flow in Chronic Ischemic Stroke Patients

BACKGROUND: Recently, stroke therapy is focused on reperfusion therapies for restoring cerebral blood flow (CBF) and minimizing the undesired effects of neuron ischemia. However, the thrombolytic therapy to restore CBF was restricted with narrow time window. On other hands, not many patients can reach the treatment immediately after the onset of stroke. A wider time window therapy that might increase CBF would probably helpful. This study aims to investigate the CBF improvement after intra arterial heparin flushing (IAHF) therapy in chronic stroke patients.METHODS: A clinical trial was conducted with time sampling. We collected chronic ischemic stroke subjects (with stroke onset ≥30 days) within periods February-September 2015. We investigated CBF before and after IAHF treatment in 75 chronic stroke patients. The difference before and after IAHF treatment in subgroup which is classified with infarct size and lesion area was tested. CBF was measured using MRI Quality Arterial Spin Labeling (qASL) with region of interest around infarct lesion.RESULTS: We found a significant CBF improvement (p<0.001) around infarct area after IAHF treatment with average 10.39mL/100g/min raised. CBF improvement was found in lacunar infarct (p<0.001) and non lacunar (p<0.001), also in infarct in cortical (p<0.05), subcortical (p<0.001) and both area (p<0.05).CONCLUSION: IAHF is associated with increased CBF around infarct area and IAHF probably offers some benefit for chronic stroke.

Antioxidant Activity of Phyllanthus Niruri Extract, Rutin and Quercetin

BACKGROUND: Normal metabolism of oxygen and exogenous factors constantly generate free radicals which could be harmful to the human body. Human need antioxidants to provide protection against free radicals, thus plants are a good source of natural antioxidants. Phyllanthus niruri (P. niruri) has been known to possess several medicinal properties and contain numerous active phytochemical. In this research, we conducted phytochemical screening and antioxidant assay of P. niruri extract along with the compounds rutin and quercetin, which are flavonoids possessing medicinal properties. This study was conducted to determine P. niruri, rutin and quercetin as antioxidant.METHODS: In this study, qualitative phytochemical screening was performed to detect phenol, flavonoid, saponin, tannin, steroid/triterpenoid, terpenoid and alkaloid in P. niruri extract. Antioxidant analysis of P. niruri, rutin and quercetin was conducted using total measured phenolic content, 2,2-diphenyl-1-picrylhydrazil (DPPH), 2,2'-azinobis-3-ethylbenzo-thiazoline-6-sulfonic acid (ABTS) and ferric reducing antioxidant power (FRAP) assays.RESULTS: The study revealed that P. niruri extract contained saponin, phenol, flavonoid and tannin based on phytochemical screening. In DPPH and ABTS assays quercetin possessed highest antioxidant activity with IC50 value of 0.55 and 1.17 μg/ml respectively. Meanwhile, P. niruri extract showed the highest FRAP activity which was 373.95 μM Fe(II)/μg extract. Rutin possessed the lowest antioxidant activity in all antioxidant assays.CONCLUSION: This study confirmed that P. niruri extract and quercetin have great potential as a natural antioxidant source.

Concentration of Endogenous Secretory Receptor for Advanced Glycation End Products and Matrix Gla Protein in Controlled and Uncontrolled Type 2 Diabetes Mellitus Patients

BACKGROUND: Advanced glycation end products (AGE) and their receptor (RAGE) system play an important role in the development of diabetic vascular complications. Recently, an endogenous secretory RAGE (esRAGE) has been identified as a novel splice variant, which lacks the transmembrane domain and is secreted in human sera. Interestingly, it was reported that esRAGE binds AGE ligands and neutralizes AGE actions. Many studies have reported that diabetes mellitus correlates with vascular calcification event and increases progressively in uncontrolled diabetes. Matrix Gla Protein (MGP) is known to act as an inhibitor in vascular calcification. The aim of this study was to observe progress of vascular calcification in uncontrolled diabetes patient by biochemical markers MGP as inhibitor in vascular calcification, via mechanism of AGEs.METHODS: This study was an observational study with cross sectional design on adult type 2 diabetic male patients who were defined by the 2011 Indonesian diabetes mellitus consensus criteria.RESULTS: The results of this study showed that there was a positive significant correlation between esRAGE and HbA1C (r=0.651, p=0.009), and negative correlation between MGP and HbA1C (r=-0.465, p=0.081) in controlled diabetes group. In uncontrolled diabetes group there was a positive significant correlation between MGP and HbA1C (r=0.350, p=0.023), despite the fact esRAGE showed no significant correlation with HbA1C. There was no significant difference in level of esRAGE and MGP in controlled and uncontrolled diabetes group, but MGP showed lower level in uncontrolled diabetes group, contrary to esRAGE that had higher concentration.CONCLUSION: In diabetes condition, complications of vascular calcification are caused by the mechanism of increased AGE formation represented by esRAGE. In diabetes control it is very important to keep the blood vessels from complications caused by vascular calcification.
